Initiation Form
To order initiation materials, cords, banners, badges and/or replacement charters, click here . This online form must be completed for all orders including those being paid for with credit card or check. Membership fees are $50 per member and include the following:
- Initiation Certificate
- Pin
- Graduation Cord
Note all memberships are non-refundable.
Be prepared to upload a spreadsheet with your initiates’ information. Use the following column headers: First Name, Last Name, Affiliation, and Email Address.
