The preferred way to submit your initiation forms is electronically, you can input names, place the order and be invoiced electronically to pay via a secure server or mail a check into the office. This allows for the most accurate processing possible. Please note no orders will be placed until payment is received.

**All Fraternity/Sorority members listed on an initiation roster must be in good standing with his/her chapter, has fulfilled the academic eligibility requirement of Gamma Sigma Alpha (3.5 cumulative GPA or higher out of 4.0 as a junior 160 or more units) or 3.5 or higher out of 4.0 in any junior or senior semester), and has submitted the $35 per inductee national membership fee.
